Chest Trauma with Hemothorax and/or Pneumothorax


Abstract
1. This report consists of clinical review of 62 cases of chest trauma seen and treated at 1st ROK Mobile Surgical Hospital, Vung-Tau, Viet-Nam during the past two years from November 1964 to Sept-ember 1966.
2. The trauma was caused by penetrating wound in 57 cases (91.5%) and 41 cases had associated injuries to abdominal organs, nervous and vascular system, bones and soft tissue.
3. Closed thoracotomy was carried out for 52 cases(83.5%) in early postinjury period and on six cases repeated thoracenthesis was carried out. Open thoracotomy was not carried out since there was no case requiring it. It is proposed that unless there are clear cut indications for the open thoracotomy in chest trauma, treatment should be directed toward meticulous supportive therapy and closed thoracotomy. Indication for the open thoracotomy has been discussed.
4. The most common complication was intrathoracic infection and they were treated conservatively without significant morbidity. The only case requiring surgical intervention had paravertebral abscess . which was drained surgically together with removal of foreign body retained there.
5. Overall mortality was 6.4 f/ and principal cause of death was hemorrhagic shock incident to trauma and associated head injury, and one case was considered to be due to myocardial infarction although not verified by autopsy.
